Yogi Rehab is a credentialed provider for Medicare B with a option for out of network reimbursement. We handle the benefits verification process for you. We Instantly verify your benefits online or by phone. All other services will be self pay with a option for out of network reimbursement. 

Self pay charges for mobile physical therapy:

• Initial in person PT Evaluation:$200 & follow up in person PT appointments:$150/ visit 

• Initial Telehealth virtual PT evaluation:$130 & follow up virtual PT appointment: $90/visit

• Fitness-Wellness: 45 mins session : $110/session 

• Yogi Monthly Plan (YMP) - Pay $792 monthly and get eight , 45 mins fitness/wellness sessions. (non refundable and non transferable to next month or member)

• PT savings account (PSA) for mobile Physical Therapy & Wellness Services: 

• You get 10% off all your visits & evaluations when you pay $3080 in your PSA account and your charges get deducted as you use the services.
