Yogi Rehab is a credentialed provider for Medicare B, CareSource, Anthem, Aetna, Buckeye Health, Medical Mutual all other services will be self pay with a option for out of network reimbursement. We handle the benefits verification process for you. We Instantly verify your benefits online or by phone. All other services will be self pay with a option for out of network reimbursement. 

Self pay charges:

• Initial in person PT Evaluation:$150 & follow up in person PT appointments:$110/ visit 

• Initial Telehealth virtual PT evaluation:$130 & follow up virtual PT appointment: $90/visit

• Fitness-Wellness: 45 mins session : $100/session 

• Yogi Monthly Plan (YMP) - Pay $760 monthly and get eight , 45 mins fitness/wellness sessions. (non refundable and non transferable to next month or member)

• Plans for half hour kinesio taping session with manual work :

• Call in one visit , no commitments - $ 80 plus the cost of kinesio tape

• The flexible 5 pack - 12 months to use - $ 350 plus the cost of kinesio tape ($50 savings compared to the call in)

• The regular -1x/month - $80/session cost of tape waived (4 month minimum/automatic withdraw)

• 2x/ months - $70/session cost of tape waived (4 month minimum/automatic withdraw)

• PT savings account (PSA): You get 10% off all your visits & evaluations when you pay $2520 in your PSA account and your charges get deducted as you use the services.
