For Mobile PT services Yogi Rehab is a credentialed provider for 

Medicare B with a option for out of network reimbursement. We handle the benefits verification process for you. We Instantly verify your benefits online or by phone. All other services will be self pay with a option for out of network reimbursement. 

Self pay charges for mobile PT services 

• Initial in person PT Evaluation:$200 & follow up in person PT appointments:$150/ visit 

• Initial Telehealth virtual PT evaluation:$130 & follow up virtual PT appointment: $90/visit

• Fitness-Wellness: 45 mins session : $110/session 

• Yogi Monthly Plan (YMP) - Pay $792 monthly and get eight , 45 mins fitness/wellness sessions. (non refundable and non transferable to next month or member)

• Plans for half hour kinesio taping session with manual work :

• Call in one visit , no commitments - $ 90 including the cost of kinesio tape

• The flexible 5 pack - 12 months to use - $ 400 includes cost of kinesio tape 

• The regular -1x/month - $90/session (4 month minimum/automatic withdraw)

• 2x/ months - $85/session (4 month minimum/automatic withdraw)

• PT savings account (PSA): You get 10% off all your visits & evaluations when you pay $3080 in your PSA account and your charges get deducted as you use the services. 

• Benefits of having a PSA account: You get 10% discount on all the items in the general store under patients portal in addition to your physical therapy & fitness services with Yogi Rehab. 

•  Your money stays in PSA account until used up and it never expires and is non refundable.
