1) Do I need a prescription/referral for Physical Therapy services by Yogi Rehab? 

Medicare rules

Medicare requires a signed plan of care for physical therapy. Your Physical therapist will create a plan of care during your first visit, and our team will work with your doctor to get it approved. You will need a new plan of care established every 90 days or 10 visits whichever is sooner.

Self-pay rules

In Ohio, Direct Access rules allow patients to be seen for physical therapy without a prescription. If the patient does not demonstrate progress within 30 days, the patient must be referred to an appropriate healthcare provider.

2) How to schedule my appointment?

Click on schedule appointment on any page , it will take you to a patient portal, register your self & schedule. 

We are open 5 days / week :

Monday: 9 am - 4.30 pm

Tuesday : 9 am - 4.30 pm 

Wednesday: 9 am - 7 pm 

Thursday: 7 am - 7 pm  

Friday: 9 am - 4.30 pm 

Saturday: Closed

Sunday: Closed
